Counseling  Services

 

LP provides counseling to individuals who need assistance with any of the following issues:  death and dying, family supports, divorce, aging, substance use, mental health, depression.  Services are provided by individual, group and family

 

 Consumers are directed to community based organizations  for supportive services as needed. All major insurance is accepted, a sliding fee. 

 

 

Community Re-entry Program

 

LP has been assisting individuals with their Community Re-entry Program from the legal system.  The goal of this program is health education and directing members to establish relationships with social service agencies while adjusting to living in the community.   This consists of  facilitating groups on basic health education in a 5- week Health Education Series.  Topics in the series are:

 

• Diabetes 101

• High Blood Pressure

• Healthy Eating and Nutrition

• Substances - Man in the mirror

• Healthy Relationships and Strengths 

 

*All groups provide consumers with vital information about various social service agencies to address their needs.
